New Patients

Patients moving into a new area should register with a new Doctor as soon as possible. Please contact the surgery to make appropriate arrangements. Talk to reception, they will give you a form to complete (GMS1) and an appointment time to see the Health Care Assistant.

Please bring the following with you when registering:

  • Urine sample
  • Proof of identity (e.g. Driving Licence, passport)
  • Proof of address (e.g. Utility Bill, Council Tax)

All new patients over the age of 16 are invited for a routine health check on registering. This is normally carried out by our Health Care Assistant and includes being asked about past illnesses and family problems. You will be given an examination for weight, blood pressure, etc.

A Health Visitor will contact parents of under fives. You will be registered with both doctors but you have the right to express a preference of a GP if you wish, please ask reception.

If you need help and are away from your usual Doctor, we are able to see you at the times outlined

Named Accountable GP

From 1st April 2015 onwards Practices are required, under the GMS contract, to allocate a named accountable GP to all patients, including children. It is important that you are aware that this is purely an administrative exercise and does not change the way in which we operate or affect your ability to make an appointment with any of the GPs in the practice. We have now allocated all patients to a named GP and all new patients registering will be allocated within 21 days of registration.

If you were registered with us before 1st April 2015 and were aged over 75 or are taking part in the Unplanned Emergency Admissions service provision, where you were allocated a care plan coordinator,  you will have already been allocated a named accountable GP and notified in writing – this named GP will remain the same and will not change.

Should you express a wish to change your named GP we will do our best to accommodate your wishes, but this cannot be guaranteed due to the different workload of our GPs who work full time and part time.  Please write to the Practice Manager with your request.
